A11 Claim deleted
A11A Claim 011901 has been deleted. There is a communication in the post.
B11A Claim already processed on schedule 1409, therefore this apparent duplicate claim has been withdrawn.
B11B Claim currently awaiting processing therefore this apparent duplicate claim has been withdrawn.
B14 Before retransmitting please AMEND CLAIM AS REFERENCE NOT APPLICABLE WITHOUT APPROPRIATE ENTRY IN TREATMENT FIELD, OR REMOVE REFERENCE.
B15 CLAIM **** RETURNED TO YOU IN ERROR. PLEASE RETRANSMIT. OUR APOLOGIES.        
B16 Observations noted, but due to the age of this claim the NHS Dental Services are unable to process it.
B2 It is not possible to process your request to delete this patient's original form and substitute it with this replacement form. The NHS Dental Services is unable to reassign UDAs between performers under the same provider, as units of activity are allocated at provider level.
